Consent for Photography and Use

I voluntarily consent to the taking of photographs and/or videos before, during, and after my treatment at Dr. Don Aesthetics. I understand and agree that:

1. The images and/or videos may be used for educational, informational, and marketing purposes, including but not limited to:

   • Social media platforms (Instagram, Facebook, TikTok, YouTube, etc.)

   • Website and digital media

   • Educational presentations and training materials

   • Printed promotional materials

2. My identity will be protected as much as possible. The clinic will take reasonable steps to anonymize images (e.g., blurring facial features or excluding identifying marks) if requested.

3. I may request to review any images before they are shared publicly, and I can withdraw my consent at any time by providing written notice. However, I understand that once content has been published online, complete removal cannot be guaranteed.

4. This consent is voluntary, and my decision will not affect my treatment in any way. I understand that refusal to consent will not impact my medical care.

5. I release Dr. Don Aesthetics, its staff, and affiliates from any claims or liabilities related to the use of my images in accordance with this consent form.
